UIC Nutrition Application

Current UIC students may apply to the UIC College of Applied Health Sciences for the Bachelor of Science in Nutrition (Coordinated Program) or Nutrition Science. Students should apply as inter-college transfer applicants when most of the prerequisite courses have been completed. All prerequisite courses must be completed with a grade of C or better no later than the semester before planned entry (for example, spring 2019 for fall 2019 entry in the Bachelor of Science in Nutrition, Coordinated Program major). To determine if you are ready to apply please consult your assigned pre-health advisor.

The Bachelor of Science in Nutrition has its own application along with supplemental forms, documents, and letters required for admission.

Current UIC students should complete the intercollege transfer application rather than the undergraduate application. Please see the Pathways to Nutrition if you are unsure which nutrition major might be the best option for you. Students are also encouraged to meet with their pre-health advisor to discuss their options.

The Nutrition Science major accepts students for both fall and spring entry. The deadline for applicant who wish to start in the spring semester is October 1. The application deadline for fall start is January 15. The Nutrition, Coordinated Program major accepts students for a fall entry only. The application deadline is January 15.

Both the Nutrition Science program and Nutrition, Coordinated Program require that applicants complete the Prerequisite Form. This form will ask you to list each prerequisite course as well as when and where it was taken. If you have courses in progress or that you plan to take in a future semester, you should include when and where you plan to take the course(s).

Current UIC students do not need to submit a transcript from UIC. Any coursework from other institutions that appears on your UIC transcript does not need a separate transcript. However, if you have completed coursework that does not appear on your UIC record, you will need to submit an official transcript from that institution with your application.
